• Acquired Immune Deficiency Syndrome [AIDS]

• Hepatitis C

• Amyotrophic Lateral Sclerosis [ALS]

• Crohn's disease

• Agitation of Alzheimer's disease

• A chronic or debilitating disease or medical condition or the treatment for a chronic or debilitating disease or medical condition that causes: Cachexia or wasting syndrome, severe and chronic pain, severe nausea, seizures, and severe or persistent muscle spasms, including multiple sclerosis.

If a patient does not see their condition in the required list, the new law in Arizona states that a request can be made for the addition of other conditions to the qualifying list. These requests can be submitted twice a year, in January and July. The requests must be written and submitted to The Department of Health Services. To have a condition approved the following guidelines should be adhered to:

1. State the condition or disease.

2. Explain the symptoms and how it negatively affects daily living.

3. List the availability of conventional medicine.

4. Summarize how medical marijuana will provide relief or comfort to the condition.

5. Include any articles or scientific journals reporting research on the effectiveness of treating the condition with cannabis.
